Medical Information Form (MEDIF)

 

 

Prior to travel, passengers with health concerns or requiring travel assistance must confirm their eligibility to fly.  Completion of the Medical Information Form (MEDIF) allows our team to assess travel safety and identify any necessary support.

Kindly request your physician to complete the form and email it to Ops@aeromedairrescue.com.

Commercial Medical Escort

Our medical escort service is tailored to patients capable of seated travel and possessing sufficient mobility to transfer, with assistance, between a wheelchair and a commercial airline seat.

 

These patients must exhibit medical stability, with needs limited to oxygen administration, intravenous/oral medication administration, mobility assistance, and/or orientation support for controlled mental health conditions.

 

Our service encompasses transportation from the patient's origin (home or hospital) to their final destination. Aeromed provides global medical escort services.  

Attending medical personnel—a physician, paramedic, or registered nurse—are assigned based on the patient's specific needs.

The escort collects the patient from their home or hospital, accompanies them on a commercial flight, and provides necessary basic medical care and support throughout the journey.

Aeromed manages all aspects of the transfer, including airline coordination, medical personnel arrangements, oxygen and medication provision, and ground transportation (ambulance or private car).

Our pricing is all-inclusive.  Before transport, our medical director consults with the patient's physician to confirm that medically supervised air transport is the optimal solution.  Final approval rests with the airline medical department.

Aeromed Air Ambulance is an indirect air carrier authorized under U.S. Department of Transportation Order 83-1-36, 99 C.A.B. 801 (1983), and uses the services of licensed FAA Part 135 direct air carriers or foreign equivalent carriers, to meet the air ambulance or medical evacuation transportation needs of its clients. Aeromed Air Ambulance does not own, lease or operate any aircraft. As an indirect air carrier, Aeromed Air Ambulance contracts for the provision of air transportation services in its own name and coordinates the provision of medical services. All flights are operated by licensed direct air carriers. Aeromed Air Ambulance must make all the arrangements for Air Medical Transport Services.
